Getting Started

How does the eligibility process work?

Start by completing a brief, HIPAA‑compliant intake. After the first 5 questions, you’ll see instant eligibility results.

If you’re eligible, you’ll answer a few additional questions so your physician can review your information and determine appropriate next steps.

Does it cost anything to check eligibility?

No. There is no payment required to check eligibility or to receive your initial eligibility result.

How long does it take?

No. There is no payment required to check eligibility or to receive your initial eligibility result.

What happens after I submit?

After submission, your information is reviewed in your secure patient portal. You’ll receive clear next steps in the portal once the review is complete.

If a live conversation is clinically needed, a member of the clinical team will contact you.

Why do you ask for a credit card?

If you’re eligible and choose to continue, we may ask for a credit card before final submission so billing can be handled smoothly if you move forward with a recommended plan.

Your card will not be charged until your submitted information has been reviewed by a physician and the next steps are confirmed.

Can I speak to someone if I have questions?

Yes. Our support team is available by phone at 1‑888‑455‑3411 and by email at hello@praxiomhealth.com.

Clinical calls are used when medically needed; otherwise, next steps are typically provided through your secure portal.

Can I save my progress and come back later?

If the intake supports saving your progress, you can pause and return later to finish when it’s convenient

Will you email or text me if I don’t finish?

If you start the intake and share your contact information, we may send a reminder email to help you continue where you left off. If you opt in to SMS, we may also send text reminders. You can unsubscribe from reminders at any time.

No payment required • HIPAA‑compliant • Instant eligibility after the first 5 questions

Care Model

Is Praxiom physician-led?

Yes. Praxiom is physician-run, and your information is reviewed by a licensed physician before any medication is prescribed. Our care model is designed around safety, individualized decision-making, and ongoing oversight.

Is my “monthly consultation” a phone call?

Not always. In most cases, monthly oversight is completed as a physician review of your information in your secure portal (your progress, reported side effects, and next steps).

If a live conversation is clinically needed, your physician (or the clinical team) will contact you.

What does “higher-touch support” mean at Praxiom?

Higher-touch means you’re not left to figure things out alone between check-ins. Patients have access to support for questions, clarification on next steps, and help navigating the process—so care feels guided, not transactional.

Can I get support between monthly reviews?

Yes. Support is available between monthly physician reviews through your secure portal and through our toll-free number during business hours.

How do I contact support?

You can contact our team by phone at 1‑888‑455‑3411 or by email at hello@praxiomhealth.com.

Business hours: Mon-Fri, 9-5 ET

How quickly will I hear back?

We aim to respond as promptly as possible during business hours. Response time can vary based on message volume and clinical complexity, but you’ll always see clear next steps posted in your secure portal.

Medication and Treatment

What medications do you prescribe?

When medically appropriate, Praxiom may prescribe FDA‑approved brand‑name GLP‑1 medications and/or compounded formulations. All medication decisions are made only after a physician reviews your health history and current risk factors.

Do you prescribe compounded GLP‑1 medications?

In some cases, yes—when clinically appropriate. Whether a compounded option is appropriate depends on your medical history, current medications, goals, and other clinical factors.

Are compounded medications FDA‑approved?

No. Compounded medications are not FDA‑approved. Your physician will review your situation and discuss options and next steps through your secure portal.

Where do medications come from?

When medication is prescribed, it is fulfilled through state‑licensed U.S. pharmacies. Pharmacy availability may vary by state and medication type.

How do you decide which medication is right for me?

Your physician considers factors such as:

  • Your medical history and current medications
  • Prior weight management history and goals
  • Contraindications and risk factors
  • Tolerability and side‑effect considerations

The goal is to recommend an approach that is clinically appropriate and safe, not one-size-fits-all.

Is medication included in the monthly package?

Medication cost is included in monthly packages. Your physician will explain the proposed plan and any associated costs before anything is charged.

Are labs included?

No. Labs are not included. In some situations, labs may be recommended at additional cost when clinically appropriate.

Do you treat other conditions or only weight management?

Praxiom’s program is focused on weight management and metabolic health. If something falls outside our scope of care, we’ll let you know and may recommend next steps.

Payment and Insurance

When do I pay?

The eligibility check is free, and there is no payment required to receive your initial eligibility result.

If you’re eligible and choose to continue, you may be asked for a credit card before final submission. Your card will not be charged until your submitted information has been reviewed and you’ve been presented with the proposed plan and total amount.

Why do you ask for a credit card?

We collect a card near the end of intake to make checkout and onboarding smoother if you decide to move forward.

We do not charge your card until:

  • Your information has been reviewed, and
  • You’ve been clearly informed of the proposed amount (including what’s included)

What is included in the monthly package?

Your monthly package includes:

  • Monthly physician review (high‑touch oversight)
  • Medication
  • Shipping (flat rate)
  • Pharmacy handling fee ($10) to coordinate fulfillment so you don’t have to manage pharmacy logistics directly

Once approved, our team will walk you through the package cost before charging your card.

Do you accept insurance?

At this time, Praxiom’s program is primarily cash‑pay. If insurance options become available for certain situations, we’ll explain eligibility and next steps clearly.

Are there multi‑month plans or discounts?

In some cases, Praxiom may offer multi‑month plans (e.g., 90‑day, 180‑day, or annual) at a discounted rate compared to paying month‑to‑month.

For current options and details, please see our Pricing page.

Will you charge my card if I’m not approved?

If, after physician review, medication is not recommended, you may be charged a reduced physician review fee ($24.99) for the time spent reviewing your information and communicating next steps.

If medication is recommended and you choose to proceed, the physician review component is $74.99 as part of the package. You’ll be walked through the proposed total before anything is charged.

What if I want brand‑name only?

Tell us during intake or message the clinical team in your portal. Your physician can explain which options may be appropriate for you based on your medical history, availability, and your preferences.

Safety

Are GLP‑1 medications safe?

When prescribed appropriately and monitored correctly, GLP‑1 medications are generally considered safe and effective for many patients. However, they are not appropriate for everyone. That’s why Praxiom requires physician review and ongoing oversight before and during treatment.

What side effects are common?

Some people experience side effects, especially early on or after dose changes. Commonly reported side effects can include nausea, constipation, diarrhea, decreased appetite, and stomach discomfort.

Your care team will help you understand what’s expected, what can be managed with plan adjustments, and when to escalate concerns.

What should I do if I’m having side effects?

Message the clinical team through your secure portal with what you’re experiencing, when it started, and whether it’s getting better or worse. If a live conversation is clinically needed, your physician or the clinical team will contact you.

If you feel you may be experiencing a medical emergency, call 911 or go to the nearest emergency room.

What if I have existing health conditions?

Many patients have underlying conditions (for example, metabolic risk factors or hormonal conditions). Your physician will review your medical history, current medications, and risk factors to determine whether treatment is clinically appropriate and what monitoring is needed.

If something falls outside our scope of care or isn’t safe to manage through our program, we’ll tell you and recommend next steps.

Who may not be a good fit?

The program may not be a fit if treatment is not clinically appropriate based on your medical history, current medications, or risk factors. The eligibility and physician review process is designed to identify these cases early and prioritize safety.

What happens if medication isn’t recommended for me?

If a physician determines medication isn’t appropriate, we’ll communicate clear next steps in your portal. In some cases, we may recommend alternative actions or follow up with an in-person clinician depending on your situation.
